What’s included:
Wellness & Routine Check (Every Visit)
We check in face-to-face to make sure your loved one is doing well and staying on track with daily routines. We look for changes that matter — including mood, energy, confusion, or anything that feels “off.”
Home Safety Check
We do a quick safety scan of the home environment to spot obvious hazards or concerns, and we notify the family immediately if we see anything urgent.
Food Check
Food and nutrition are a priority in this plan. During visits, we:
Confirm your loved one is eating and has a plan for upcoming meals
Check pantry/fridge basics to ensure safe, usable food is available
Identify expired food items and flag them (and remove them with permission)
Note what’s running low so the family can restock quickly
Family Updates
After each visit, you’ll receive an update so you’re always informed. If something needs attention, we’ll reach out promptly so you can take action fast.
Important Note: AssistedLink provides non-medical support and oversight. We do not provide medical care or hands-on personal care. Our goal is consistent presence, practical monitoring, and early identification of concerns before they become emergencies.
